Mr. Boateng showed me child monitoring forms and how to fill them using information from the Census Summary Form which I previously worked on. This form had information on the Child’s Identification Number, Name, Community, Education, etc. Please find a copy of the monitoring form below:
He further explained to me that the monitoring forms are to be filled in duplicate so that whereas a copy is kept on the file of the child at school, another copy is also kept at the Customer Relations Office for external auditing and other uses as the need arises. Afterwards, I was given enough forms to record information on each child from the census summary form provided.
